The story begins six days after the tragic death of Dr. Rose Cotter, the central figure in the first film. Joel, the cursed police officer, desperately tries to rid himself of the Smile Entity. He attempts to pass the curse by orchestrating a murder where one criminal kills another. However, during a chaotic shootout, Joel accidentally kills both, failing to transfer the curse. A bystander, Lewis Fregoli, witnesses the event and inherits the curse instead. Believing himself free, Joel attempts to flee but is struck and killed by a truck.
Meanwhile, pop star Skye Riley is gearing up for her highly anticipated comeback tour. Still haunted by past traumas, including a public battle with substance abuse and the death of her actor boyfriend Paul Hudson in a car crash, Skye tries to focus on her career. Despite her support system—comprising her assistant Joshua and her mother-turned-manager Elizabeth—Skye begins to crack under the pressure. During a rehearsal, she injures her back and sneaks out to buy Vicodin from Lewis, an old schoolmate turned drug dealer.
Their meeting takes a horrifying turn. Lewis, under the influence of the Smile Entity, becomes erratic and violently kills himself with a barbell plate. Panicked and terrified of being implicated, Skye flees without alerting anyone to the gruesome scene.
Following this event, Skye's life spirals into chaos. She begins experiencing vivid and terrifying hallucinations of people smiling at her in unnatural ways. In her desperation, she reconciles with her estranged friend Gemma, seeking support and comfort.
Skye receives a cryptic text from an unknown number, warning her of the danger she's in and revealing knowledge of her presence at Lewis's apartment. Later, at a fundraiser hosted by music executive Darius, Skye hallucinates again. She believes the teleprompter has stalled and goes on a rambling, emotional tirade about how success has failed to bring her peace. Her outburst escalates when she sees a smiling vision of Paul in the audience, causing her to panic and accidentally injure an elderly guest.
Skye meets Morris, the man who sent her the text. He explains that he has been tracking the Smile Entity since it killed his brother. Morris reveals that the curse passes to anyone who witnesses a victim's gruesome death, and theorizes that stopping Skye's heart and reviving her might break the curse. Fearful and skeptical, Skye refuses his plan and storms out of the meeting.
As the Entity's influence grows stronger, Skye's hallucinations become more violent. One night, in her apartment, she is attacked by the Entity, which manifests as her backup dancers. After a terrifying struggle, Skye is thrown around her apartment and rendered unconscious.
A flashback reveals the full extent of Skye’s guilt over Paul’s death. During a drug-fueled argument, she intentionally crashed their car, killing him. This trauma adds another layer to her unraveling psyche. At a wellness retreat meant to prepare her for her tour, Skye becomes embroiled in a horrifying event. The Entity manipulates her into believing her mother, Elizabeth, has become cursed. In a moment of confusion, Skye witnesses Elizabeth smashing a mirror and stabbing herself. However, to her horror, she realizes she was the one who committed the act under the Entity’s influence.
Skye escapes and reconnects with Gemma. Together, they steal a car to meet Morris, intending to try the heart-stopping plan. While driving, Skye receives a call from the real Gemma and realizes the Entity has been impersonating her friend all along. After regaining control of the car, Skye arrives at an abandoned Pizza Hut to meet Morris.
The Entity confronts Skye, taking the form of her younger self. It mocks her attempts to fight back. Desperate, Skye injects herself with the syringe intended to stop her heart, hoping to end the curse. However, the Entity remains unaffected, affirming that Skye has no control.
The final act sees Skye transported to a nightmarish hallucination where she performs on stage at Herald Square Garden. The Entity reveals its true form: a grotesque, skinless creature with countless smiling mouths. The Entity forces itself inside her, leaving the horrified crowd to witness Skye collapse. Possessed, she stands up smiling and stabs herself in the eye with her microphone, passing the curse to the thousands of spectators.
